Priority Service Areas
- At the Phoenix Center we make every effort to insure that the needs of the community are met. We not only take a proactive approach to planning for the future in our annual strategic plan, but work hard to keep the citizens of Greenville and surrounding areas informed of our priority service areas.
Client Rights
As a client of The Phoenix Center you are entitled to the following rights, which may not be revoked for any reason:
- Respectful treatment in a dignified, secure and private manner which includes respect for property
- Privacy during treatment, visits, and while receiving personal care
- Confidential services that are protected by Federal Law with specific restrictions in regard to the use, storage and disclosure of any record of participation in this program
- Access to all services regardless of race, sex, national origin, physical or mental handicap, gender, age, sexual orientation, or personal ability to pay
- Professional services which do not include any form of abuse, neglect or humiliation as part of the treatment model
- Protection from financial or other exploitive activity
- Provision of care in the least restrictive environment
- Explanation of grievance procedures to include the address and phone number of SC DHEC, access to forms and other resources to initiate grievance, receive a decision in writing, and appeal to unbiased sources
- Protection from retaliation in the event of a grievance or other complaint against a staff member being filed
- Access to records and timely response to requests for copies of records
- Protection from the behavioral disruptions of other clients
- Timely access to information necessary to make decisions in regard to treatment including evidence based information about alternative treatments, medications, and modalities
- Informed consent, refusal, or other expression of choice regarding service delivery, release of information, concurrent services, composition of the service delivery team and involvement in research projects

- Adherence to research guidelines and ethics
- Complete and timely investigation and resolution of alleged infringement of rights or other inappropriate staff activity
- Individual evaluation and treatment recommendation based on strengths, needs, abilities and preferences, with full opportunity to participate in the Individual Plan of Care
- Assessment of fees and development of payment plan in a manner that is fair and consistent
- To refuse treatment or withdraw from services at any time without affecting re-entry at a later time; however, the Treatment Team may place time limits for re-entry on clients who are not making progress and refusing other services. This time frame is generally between 1-6 months unless the individual is in crisis or can demonstrate a commitment to recovery and the helping process

Confidentiality
- Federal laws and regulations protect the confidentiality of alcohol and other drug abuse clients and client records maintained by The Phoenix Center. The Phoenix Center may not say to a person outside the agency that a client attends the program, or disclose any information identifying a client, unless:
- The client consents in writing.
- The disclosure is allowed by court order, accompanied by a subpoena, specifically authorizing disclosure and its use.
- The disclosure is made to personnel for research, audit, program evaluation or medical personnel in a medical emergency.
- Federal Confidentiality Laws do not protect any of the following information from being reported under State law to appropriate State or local authorities:
- A crime committed by a client either at the program or against any person who works for the program
- Any threat to commit such a crime
- Any information about suspected child abuse or neglect
Record Requests
- The Phoenix Center maintains electronic and some paper client records. Clients may submit a request to review, copy, or make amendments to their treatment records or to have a copy of their records sent to other specified individuals or organizations.

- The Phoenix Center is prohibited under Federal Alcohol and Drug Abuse Confidentiality law, 42 C.F.R. Part 2 from responding to a records request or from confirming or denying that an individual is receiving or has received services from The Phoenix Center without the proper consent.
Clients must complete the Release of Confidential Information Form prior to the request being completed when the information is to be released to anyone other than the client.
- When possible, a summary will be provided to the requestor instead of a separate copy of each individual document.
- A fee is charged when copies are made at the request of a client or a third party with which the Agency does not have a Memorandum of Agreement. Charges include:
- A $15 administrative fee for each copy of the chart up to 10 pages to cover searching and handling plus the cost of postage.
- A charge of $.40 for each additional sheet.
- Client payment is required before the copy of the file is released. For third party requests that we do not have a MOA, an invoice is sent along with the copy of the file and is required to be paid upon receipt.
- Clients may receive a copy of their Discharge Summary and drug screen results at no cost. Clients will need to provide a picture ID when picking up records. Records may be mailed to the last known address on file.
